ILIT is a newer approach to allergy immunotherapy where tiny doses of allergen are injected directly into a lymph node rather than under the skin. Because lymph nodes are where the immune system learns and adapts, this targeted delivery may help your body build tolerance faster — with far fewer injections than traditional allergy shots. This handout explains what ILIT is, who it's for, what to expect, and what it costs.
1 What Is ILIT?
ILIT stands for intralymphatic immunotherapy. Like traditional allergy shots, it works by exposing your immune system to small amounts of the things you're allergic to — but the method is different. Rather than injecting allergen under the skin, a provider uses ultrasound guidance to deliver the dose directly into a lymph node in the groin area.
💡 Why the Lymph Node?
Lymph nodes are the immune system's "training centers." Delivering allergen there may help your immune system become tolerant more efficiently — potentially achieving in 3 visits what traditional shots work toward over several years.
ILIT is used for environmental (inhalant) allergies:
🌳 Tree pollens
🌿 Grass pollens
🍂 Weed pollens
🏠 Dust mites
🐱 Pet dander
🍄 Mold
✗ Not for food allergies
2 ILIT vs. Traditional Allergy Shots
Both treatments work toward the same goal — a less reactive immune system — but the path is very different.

3 Before You Start
Before beginning ILIT, your allergist will take several steps to make sure the treatment is right for you.
- 1Review your medical history, current medications, and any other health conditions
- 2Confirm your allergies with skin testing or blood tests
- 3Decide which allergens to include (e.g., specific pollens, dust mites, or pet dander)
- 4Discuss benefits, risks, and alternatives — including traditional shots, sublingual tablets, or medications alone
- 5Answer your questions and review a consent form before anything begins
4 What Happens on Treatment Days
Each visit follows the same routine. Plan to be at the clinic for approximately 45–75 minutes including your observation period.
- 1Check in — a staff member reviews your current symptoms and vital signs
- 2The skin over the groin/lower abdomen area is cleaned and prepared
- 3Ultrasound is used to locate the lymph node precisely before the injection
- 4A small needle delivers a tiny amount of allergen — the injection itself takes only a few seconds
- 5You remain in the office for 30–60 minutes of observation so staff can watch for any reactions and treat them immediately if needed
Plan ahead: You must stay for the full observation period after each injection. Please arrange your schedule accordingly.
5 What Results Can You Expect?
Results vary between patients, but many people with pollen and dust mite allergies report meaningful improvement. Clinical studies — particularly in patients with grass and birch pollen allergies — found that ILIT improved symptoms and quality of life, with benefits lasting into subsequent allergy seasons after just the short treatment series.
✅ Many patients experience
- Less sneezing, congestion, and itchy/watery eyes during allergy season
- Reduced need for daily antihistamines or nasal sprays
- Benefits that continue after the series is complete
- A shorter commitment than years of weekly shots
⚠️ Realistic expectations
- Not everyone responds the same way
- You may still need some medication during peak allergy seasons
- Long-term durability data is still being studied
- This is a newer treatment — not all providers offer it
6 Side Effects and Risks
Most side effects are mild and temporary. As with all forms of allergy immunotherapy, serious reactions are rare but possible — this is why ILIT is always done under medical supervision.
Common & Mild
- Soreness, pressure, or bruising at the injection site
- Mild swelling or tenderness in the groin area
- Temporary increase in allergy symptoms (e.g., congestion or sneezing)
Less Common & Serious
- Hives, wheezing, or difficulty breathing
- Anaphylaxis — a severe, potentially life-threatening allergic reaction (rare)
⚠️ After you leave the clinic: If you develop trouble breathing, swelling of the lips or throat, dizziness, or widespread hives — call 911 or go to the nearest emergency room immediately. Do not wait to see if symptoms improve on their own.
7 Who May Not Be a Good Candidate
Your allergist will review your full situation. ILIT may not be appropriate if you:
- 🫁Have severe or poorly controlled asthma
- ⚠️Have had a serious reaction to allergy shots in the past
- 🤰Are pregnant or planning pregnancy during the treatment course
- 💊Take beta-blockers or certain other medications that can interfere with treating allergic reactions
- 🩺Have significant immune system problems or other serious medical conditions
8 Cost of ILIT
ILIT is typically offered as a self-pay package that covers all three injections, ultrasound guidance, allergen preparation, and post-injection monitoring. Costs vary by clinic.
$3,000–
$3,500
Typical range for the full 3-injection series
3 visits
All-inclusive — no ongoing monthly copays
vs. years
Of weekly shots, travel, time off work or school
For patients with high-deductible plans or significant transportation or time-off costs, the total expense can be competitive with a multi-year course of traditional shots. Ask our billing team for the exact fee and what is included.
9 Insurance & Payment Options
Most insurance plans currently do not cover the ILIT procedure itself, as it is considered an off-label use of existing allergy extracts. However, related services — office visits and allergy testing — are typically billed to your insurance in the usual way.
💳HSA / FSA funds — many patients can use these tax-advantaged accounts to cover the ILIT package fee
📅Payment plans — ask our team about spreading the cost over several months
📋Check with your insurer in advance — ask whether any component of ILIT might be covered under your specific plan
🧾Office visits & testing before or after ILIT are billed to insurance as usual and may be covered by your benefits
10 Questions to Ask Your Allergist
- ?Am I a good candidate for ILIT given my specific allergies and health history?
- ?Which allergens will be included in my treatment?
- ?What improvement can I realistically expect, and how soon might I notice a difference?
- ?What is the exact cost at this clinic, and what does it include?
- ?Can I use my HSA or FSA? Are payment plans available?
- ?How does ILIT compare to traditional shots or sublingual tablets for my situation?
11 Key Points to Remember
Your ILIT Quick Reference
- 3 injections into a lymph node, spaced 4 weeks apart — completed in about 2 months
- Targets environmental allergies (pollens, dust mites, pet dander, mold) — not food allergies
- Ultrasound guidance is used to precisely locate the lymph node before each injection
- Most side effects are mild; serious reactions are rare but possible — always done under medical supervision
- You must stay 30–60 minutes for observation after every injection
- Usually self-pay, approximately $3,000–$3,500; HSA/FSA often accepted
- Related office visits and testing may be covered by your insurance in the normal way
- Discuss your history, expectations, and finances with your allergist to decide if ILIT is right for you
